My goal with breast augmentation is to give the patient the look they desire with an absolute minimum of scarring. Plastic Surgeon's TechniqueAlthough there are incisions under the breast, in the armpit, and at the edge of the areola, I prefer a very small incision at the edge of the nipple. When healed, this scar is barely detectable. In well over 1500 implant placements I have never lost anyone's nipple sensation. Patients often ask me how I can get the implant in through such a small incision. The answer is that the implant goes in deflated and rolled up so it can fit through the small incision. Your Breast Augmentation OptionsThe implant can be placed under the pectoralis muscle or over the muscle and under the breast gland only. There are advantages and disadventages to both. Generally, I will place the implants under the muscle, however in some situations and for some patients I will place the implant sub-glandularly. Saline implants are used for primary augmentations. Silicone gel implants can only be used for certain specific indications as outlined in the silicone gel FDA studies. Breast reconstruction following mastectomy and secondary breast augmentation procedures where saline implants are not an appropriate choice are some of the indications where silicone gel implants can be used. There are different shapes and sizes of saline breast implants. In general, they come as a smooth round implant, a textured round implant, or as a textured anatomic or tear dropped shaped implant. The most popular implant chosen is the smooth round implant. I tend to use the textured anatomic implant in women who are at least a B cup and are ptotic (droopy). I always have my patients pick the size and type of implant they want. In the office I have them try the implants on where they can pinpoint and approve of the look they are trying to achieve. I always discuss with the patients all the risks and the details of the recovery during the office consultation. In addition, there are multiple follow up visits after surgery to continually evaluate and help the patient during the recovery process.
