Dee: Dr. F...Do you specialize in all incision sites or prefer certain ones
DR Fiorillo: I use all sites but really prefer inframmary
Aquarena: Have you heard anything about the new Inamed (formerly McGhan) 410 Cohesives??
Nicole: Dr. F....have you used the new Mentor HP's yet?

DR Fiorillo: yes!!!! I love them, I use them exclusively
rystal: id prefer inframmory over infra-kneecap anyday
DR Fiorillo: I think that the high profile ones give a above the muscle look, when i put them below the muscle
Ferfy: See that is what I wanted, but he said my ribcage was too wide
Dee: Do you just use them in petite narrow chested woman or all size ribcages/ribspans?
DR Fiorillo: If you can handle any implant, you can use them
DR Fiorillo: I have been using them in most women
Nicole: Dr. F, if I have a bit of non-noticeable rippling right now, will it just get worse?
DR Fiorillo: unless they are a larger size
Ferfy: Will I show ripple if I lose weight?
Dee: can you get cleavage even if you have a large ribcage?
DR Fiorillo: maybe
DR Fiorillo: are they below the muscle
Nicole: yes
Ferfy: yes
DR Fiorillo: then you should be ok
Nicole: I can feel the rippling, but I can't see it
Nicole: Do you ever use the adjustable?
DR Fiorillo: cleavage is based on you chest bone
DR Fiorillo: I never use adjustable

Nicole: chill out Crystal...it's my turn right now.
DR Fiorillo: too much manipulation, I can usually make you the size you want the first time
DR Fiorillo: never use drains
DR Fiorillo: drains are not needed
Aquarena: I've heard that drains are sometimes needed
DR Fiorillo: I almost always do inframamary, smooth,under the muscle

Nicole: How often do you see that patients usually have to have a redo due to a leak or rupture? What is the time span usually seen for saline implants?
Aquarena: I will next week for my re-do
DR Fiorillo: rupture rate is very low
DR Fiorillo: <1%
Dee: Dr. F...what is your take on what causes CC?
Dee: what's CC rate?
DR Fiorillo: about 10 year time span
DR Fiorillo: can go up to 15 years

Jill: sounds good to me
DR Fiorillo: smooth, saline, under the muscle yields a very low cc rate
DR Fiorillo: saline
Dee: do you have an opinion on what causes CC?
DR Fiorillo: I really do not know
DR Fiorillo: know one really knows
Dee: what is YOUR CC rate?
DR Fiorillo: very low, <2%
DR Fiorillo: I do have women massage them

Kathy: Doctor, do you suggest that your patients sleep on their stomachs afterwards and if so, for how long is this recommended?
DR Fiorillo: no stomach sleeping
Nicole: UGH. I've never been able to sleep on my tummy

DR Fiorillo: I do not want the implants to move for 1 month

Dee: I've been on my tummy every night since 5 weeks which was the first time I could stand it
DR Fiorillo: side is ok
Nicole: Dr. F, is there anything done that can create more cleavage? Mine seem farther apart than other girls.
DR Fiorillo: I use a surgibra for 1 month
Kathy: i was told to sleep on it but I don't know how long I should keep doing it
DR Fiorillo: no

DR Fiorillo: cleavage is not really do to the surgeon, it is more on how you chest is
DR Fiorillo: 4-6 weeks

Ferfy: if you had a patient whose breasts were two different sizes, wouldn't you put diff cc's in to match them up?
BARB: problems???
Nicole: Yeah, like they'll bang into door jams.

Kathy: 390-420
Aquarena: Is 400 cc's a C?
DR Fiorillo: probably around 300 or so
Nicole: And how many post op visits do you usually have your patients come back for?
Dee: is that per year or since 96?
DR Fiorillo: 400 cc's usually brings u up 2 sizes

Dee: what is your specialty in plastic surgery?
Aquarena: I'm a redo 34C

DR Fiorillo: between me and my partner, we perform 1,500 procedures a year
DR Fiorillo: we do all aspects of plastic surgery

DR Fiorillo: we do a lot of breast, reductions, augmentations, and reconstructions, and lifts
DR Fiorillo: yes, rhinoplasty
Nicole: you do 1,500 BA's a year? or does that incluse all aspects?

Dee: So, if someone develops CC, then you'll do an open capsulectomy?
DR Fiorillo: yes
DR Fiorillo: I will watch it for a while

BARB: did anyone ask the trans ax question before I got here
Dee: have you ever tried any antibiotics to soften it up if it's in the early stages?
DR Fiorillo: if there is no result, I will perform an oc
Dee: no Barb

BARB: if you have CC and had trans ax incision would you go back thru that incision
DR Fiorillo: since I have been doing breast augmentation, I have always used saline under the muscle
DR Fiorillo: so I do not have a lot of women with cc's
DR Fiorillo: fortunately

DR Fiorillo: why I use the inframammary inc
BARB: ok nevermind
Dee: I have inframmary and LOVE IT
DR Fiorillo: b/c for a redo you usually need to go in thru there
BARB: I could not do that
DR Fiorillo: dee-did you get a cc?
Dee: NO, I'm 10 months post-op and NO complications
BARB: thank goodness my PS said he would go thru the trans ax incision if anything ever happened
DR Fiorillo: good for you
Nicole: I had the areola incision
Dee: some palpable rippling and visual on the outer edge of the left side, but nothing I can't live with
DR Fiorillo: he can, but it is harder
BARB: I just asked because someone asked on the forum the other day and wanted another opinion on it
Nicole: And knock on wood, have had no probs and my scar has completely faded :-D yippie!
Jill: I am almost 10 months also! Yea!
BARB: that is exactly what I was afraid of scars
Dr Fiorillo: from trans ax, it is hard to get all the way to the middle
BARB: I watched the Tebbetts tape on Trans Ax
